Crystalaire Country Club Member Referral Form

Crystalaire Country Club would like to thank you for participating in the Membership Referral Program. We would like to remind all members who refer a prospective member to register the prospective member by filling out this form. In order to receive your incentives/ rewards through the Membership Referral Program, the prospective member must be registered with the Administration Office before the prospective member submits a membership application.
Your referral must be a new candidate for membership. This name should be someone that you already know and not someone you meet at Crystalaire. If you have recently submitted a name please re-submit that name to assure that it is on file. Please fill out your information below so we know who sent us the referral.

Membership Referral Rules

Former (within the past five years) or existing members are not eligible to qualify as a new member. Existing Destiny members are not eligible to claim as a referral. Any walk-in, existing or future prospective member that has been discovered by the Membership Sales Department through various solicitations are not eligible to refer as they are already on file.
